Most organizations have AI policies. They have responsible AI principles. They have ethics committees and governance boards. What they lack is infrastructure — the operational systems that enforce policies, capture evidence, and produce audit-ready records as work occurs.
Example: Policy without infrastructure
A financial services firm adopts a policy: "All AI-generated customer communications must be reviewed by a human before delivery." Six months later, an internal audit reveals that 40% of AI-generated responses in the contact center are delivered automatically without human review. The policy exists.
